The AIDS Foundation of Chicago (AFC) mobilizes communities to create equity and justice for people living with and vulnerable to HIV and related chronic diseases. We envision a world in which people living with HIV and related chronic diseases will thrive, and new HIV infections will be rare.

We’re working to achieve these vital goals by improving health equity, preventing new cases of HIV, serving as a collaboration and knowledge center and being a bold voice for change. Each year, AFC touches the lives of 7,000 people by providing them with the things we all need to reach our full potential: a home to live in, food to eat, a strong connection to health care professionals, a ride to and from the doctor’s office, and a community of people who understand exactly what they’re going through. Our family of clients thrives because of the strength of our partner network, which makes up around 40 organizations in the Chicagoland area that provide targeted levels of care to our program participants.

Beyond helping individuals live their best lives, we’re working to transform systems to make health equity a reality in Illinois and across the U.S. Our policy and advocacy work mobilizes thousands of HIV advocates each year to make sure their elected officials hear their stories and take action.
